What this trial studies
This study investigates the use of the Diameter mobile application to support lifestyle changes in patients with Type 2 Diabetes Mellitus (T2DM). It combines real-time monitoring of glucose levels, nutrition, and physical activity with personalized coaching to enhance self-management and motivation. The primary goal is to assess the feasibility and acceptability of this blended-care intervention in secondary care settings. The study is conducted in collaboration with ZGT hospital, University of Twente, and Roessingh Research and Development Institute.
Who should consider this trial
Good fit: Ideal candidates are individuals diagnosed with Type 2 Diabetes Mellitus who are comfortable using a smartphone and are receiving treatment at ZGT hospital or primary care.
Not a fit: Patients who are dependent on renal replacement therapy or have severe general diseases or mental disorders may not benefit from this study.

Inclusion Criteria: * Diagnosed with type 2 diabetes mellitus * Being familiar with using a smartphone * Being treated in ZGT hospital or primary care Exclusion Criteria: * Dependence on renal replacement therapy * Severe general diseases or mental disorders make the participation in the study impossible * Insufficient mastery of the Dutch language * Other CGM device than Freestyle Libre.
