What this trial studies

This study investigates the use of the Diameter mobile application to support lifestyle changes in patients with Type 2 Diabetes Mellitus (T2DM). It combines real-time monitoring of glucose levels, nutrition, and physical activity with personalized coaching to enhance self-management and motivation. The primary goal is to assess the feasibility and acceptability of this blended-care intervention in secondary care settings. The study is conducted in collaboration with ZGT hospital, University of Twente, and Roessingh Research and Development Institute.

Good fit: Ideal candidates are individuals diagnosed with Type 2 Diabetes Mellitus who are comfortable using a smartphone and are receiving treatment at ZGT hospital or primary care.

Not a fit: Patients who are dependent on renal replacement therapy or have severe general diseases or mental disorders may not benefit from this study.

Inclusion Criteria:

* Diagnosed with type 2 diabetes mellitus
* Being familiar with using a smartphone
* Being treated in ZGT hospital or primary care

Exclusion Criteria:

* Dependence on renal replacement therapy
* Severe general diseases or mental disorders make the participation in the study impossible
* Insufficient mastery of the Dutch language
* Other CGM device than Freestyle Libre.
