This project will see if taking metformin before ICU admission changes survival for adults with type 2 diabetes who develop severe lactic acidosis.

What this trial studies

Researchers will review medical records of adults admitted to the CHU de Strasbourg intensive care unit between January 1, 2014 and December 31, 2023 who had lactic acidosis (pH <7.35 and lactate ≥5 mmol/L) within 24 hours of admission. Patients will be grouped by whether they had been taking metformin in the days before ICU admission and compared for ICU survival and related outcomes. The analysis will exclude patients without arterial blood gas data, unknown prior treatment, or who had therapy limitations within the first 24 hours. Results aim to clarify the association between prior metformin exposure and prognosis in this high-risk ICU population.

Who should consider this trial

Good fit: Adults (≥18 years) admitted to the CHU de Strasbourg ICU with documented lactic acidosis within 24 hours and a known pre-admission medication history including metformin use are ideal candidates for inclusion.

Not a fit: Patients without arterial blood gas measurements, with unknown prior medication history, or those who had limits placed on active therapy during the first 24 hours would not be expected to benefit from inclusion or the study's findings.

Why it matters

Potential benefit: If a link is found, clinicians could better judge risks of prior metformin use in critically ill patients and improve early management decisions.

Inclusion Criteria:

* Adult patient (≥18 years)
* Admitted to the intensive care unit of Hautepierre Hospital - Strasbourg University Hospital between January 1, 2014, and December 31, 2023
* Having experienced lactic acidosis (pH \<7.35 and lactate level ≥5 mmol/L) within the first 24 hours of hospitalization.

Exclusion Criteria:

* Patient without arterial blood gas measurements during the first 24 hours of admission
* whose background treatment is unknown and could not be determined
* for whom a limitation of active therapies was implemented before or during the first 24 hours of hospitalization
