What this trial studies

Adults with hemispatial neglect due to a right-hemisphere ischemic or hemorrhagic stroke will be enrolled at Ankara Etlik City Hospital. Participants receive mirror therapy combined with continuous theta burst stimulation (cTBS) alongside standard cognitive rehabilitation according to the study protocol. Outcomes focus on changes in neglect severity and attention-related function over the treatment and follow-up period. Common exclusions include recurrent stroke, seizures, metal implants, significant sensory deficits, cognitive impairment, psychiatric or other neurologic disease.

Who should consider this trial

Good fit: Adults aged 18–75 with a first right-hemisphere ischemic or hemorrhagic stroke occurring 7 days to 1 year earlier who have hemispatial neglect and an MMSE score of 24 or higher.

Not a fit: Patients with recurrent stroke, epilepsy, metallic brain or scalp implants, significant vision or hearing problems, premorbid dementia, psychiatric or other neurological disease, or use of cognition-affecting medications are unlikely to be eligible or to benefit.

Why it matters

Potential benefit: If successful, the combined treatment could reduce neglect symptoms and improve patients' ability to attend to the affected side in daily life.

How similar studies have performed: Previous trials using rTMS or mirror therapy alone have shown modest improvements in neglect, but combining cTBS with mirror therapy is less well studied.

Eligibility criteria

Inclusion Criteria:

1. Ischemic/haemorrhagic stroke
2. Right hemisphere lesion
3. Patients with a duration of more than 7 days but less than 1 year from the date of the event
4. Aged between 18 and 75 years
5. Patients with a Mini Mental Test Score ≥ 24

Exclusion Criteria:

1. History of recurrent stroke
2. Presence of vision or hearing problems
3. Use of medication affecting cognitive function
4. History of epilepsy
5. Pregnancy or breastfeeding
6. History of malignancy
7. Pacemaker
8. Presence of metallic implants in the brain or scalp (including cochlear implant)
9. History of premorbid dementia
10. Presence of psychiatric or additional neurological disease
