This project will try using a designated nurse practitioner and telehealth to help people aged 50–80 who currently smoke or quit within 15 years get low-dose CT lung cancer screening.

What this trial studies

This prospective cohort initiative uses electronic medical records to proactively identify people at high risk for lung cancer and contacts them by phone for eligibility confirmation and scheduling. A designated nurse practitioner (NP) will guide patients through shared decision-making, smoking-cessation counseling, and referrals, with visits conducted in person or by telehealth. Low-dose CT scans will be performed at approved centers nearer to participants' homes, and the NP will communicate results and coordinate follow-up with primary care. The program specifically aims to increase screening uptake among high-risk, racial/ethnic minority, and rural/suburban patients.

Who should consider this trial

Good fit: Ideal candidates are people aged 50–80 with a 20 pack-year smoking history who currently smoke or quit within the past 15 years and receive care within the participating SIU health system area.

Not a fit: People outside the 50–80 age range, with less than a 20 pack-year history, who quit 15 or more years ago, who cannot undergo surgery if needed, or who cannot access participating LDCT centers or telehealth are unlikely to benefit.

Why it matters

Potential benefit: If successful, the approach could increase screening rates and lead to earlier detection of lung cancer in high-risk rural and minority populations.

How similar studies have performed: Previous large trials have shown that LDCT screening reduces lung cancer mortality and separate navigation or telehealth programs have improved screening uptake, but combining an NP-led navigation model with telehealth specifically for rural and minority populations is less well studied.

Inclusion Criteria:

* All individuals 50-80 years of age
* 20 pack-year smoking history
* Currently smoke or have quit within the past 15 years:

Exclusion Criteria:

* Individuals 49 years and below or 81 years and above
* Has not smoked in 15 or more years
* Individuals who develop a health problem that makes them unwilling or unable to have surgery if lung cancer is detected.
