This will try Joshi's step combined with a buccal mucosal graft to improve urine flow and surgical outcomes for people with nearly obliterated bulbar urethral strictures.

What this trial studies
This single-center interventional program applies a non-resecting modification (Joshi's step) to augmented buccal mucosal graft urethroplasty for very narrow bulbar strictures. The technique marks the narrowest segment without excising the native urethral plate and reapproximates mucosal edges before graft augmentation. Eligible patients are those with primary, non‑traumatic, nearly obliterated bulbar strictures under 1.5 cm in length, and outcomes will focus on functional urinary results and surgical success. Patients with traumatic, redo, fully obliterated, or long segment strictures are excluded.
Who should consider this trial
Good fit: Adults with primary, non‑traumatic nearly obliterated bulbar urethral strictures where the narrowest portion is less than 1.5 cm and who have not had prior urethroplasty are ideal candidates.
Not a fit: Patients with traumatic, redo, fully obliterated, or long-segment bulbar strictures are excluded and are unlikely to benefit from this specific technique.

Inclusion Criteria: * Patients with primary bulbar urethral stricture. * Patients with non-traumatic bulbar urethral stricture. * Patients with nearly obliterated bulbar urethral stricture, with the narrowest portion of the stricture is less than 1.5 cm in length. Exclusion Criteria: * Patients with traumatic bulbar urethral stricture. * Patients with redo bulbar urethral stricture. * Patients with obliterated bulbar urethral stricture. * Patients with nearly obliterated long bulbar urethral stricture.
