What this trial studies
This intervention aims to evaluate the effectiveness of universal social needs screening, community-based service referrals, and telephonic follow-up in preventing COVID-19 transmission and addressing the secondary health effects caused by the pandemic. The SINCERE intervention will be implemented in collaboration with statewide community service providers and existing health information technology. The study focuses on improving health outcomes for vulnerable and socioeconomically disadvantaged populations by overcoming barriers to community service use through intensive follow-up and collaborative goal-setting.
Who should consider this trial
Good fit: Ideal candidates are adults over 17 years old who speak English or Spanish and have completed the referral process at designated COVID-19 testing sites.
Not a fit: Patients who are unable to communicate verbally or those living in nursing facilities may not benefit from this intervention.

Inclusion Criteria: * Adult (\> 17 years) * English or Spanish speaking * Completed the referral process at the UHealth ED, one of the COVID-19 testing sites or the Primary Children's ED and indicated both social needs and willingness to receive service low- and no-cost referrals from the United Way 211 community referral service * Able to be reached by phone during the intervention OR able to complete surveys sent by text or email Exclusion Criteria: * Those unable to communicate verbally * Those living in nursing facilities, or those who are not otherwise responsible for self-care
