What this trial studies

ADMIT is a multi-component integrated home care program that combines care coordination, social support and digital tools to manage frailty and complex chronic needs. In an interventional, hybrid implementation design, eligible participants receive the ADMIT package while comparison sites continue usual care. The primary outcome is the percentage of days spent at home, with secondary outcomes including exacerbations/crises, patient and caregiver quality of life, and experience of care. The program is delivered through primary care and community sites in Barcelona in partnership with regional health authorities.

Good fit: Ideal candidates are people aged 70 or older living at home with high morbidity or complex/advanced frailty who are dependent for basic activities or have social vulnerability.

Eligibility criteria

Inclusion Criteria:

* Living at home AND
* Adjusted Morbidity Group = 3 or 4 or high risk OR Complex or Advanced frailty or chronicity OR Home health care program; AND
* Dependent for the basic daily activities OR Social complexity (SSM-CAT positive) OR Dependency grade= 1, 2 or 3 OR Lack of social support OR Home social care program OR Living alone

Exclusion Criteria:

* Living in a nursing home
* Unable to make decisions about care (either by the patient or the caregiver)
* Person with needs that exceed the coverage of the program
* Not providing informed consent
