This project will try different ways to connect teens and young adults with type 2 diabetes and their families to social supports and resources to help with diabetes care.

What this trial studies
This pilot enrolls adolescent-caregiver dyads followed at UPMC Children's Hospital of Pittsburgh and tests several strategies to address health-related social needs, including tailored resources, universal empowerment materials, text messaging, and community health workers. Participants continue routine diabetes clinic visits, complete surveys and interviews, and are connected to community organizations as needed. The study collects process data and participant feedback to measure feasibility, acceptability, and implementation reliability. Results will guide larger programs and trials aimed at reducing social barriers to diabetes management in youth.
Who should consider this trial
Good fit: Ideal candidates are adolescents aged 13–22 with a diagnosis of type 2 diabetes who receive care at UPMC Children's Hospital of Pittsburgh and can participate alongside a consenting caregiver.
Not a fit: Patients who do not speak English or who are not followed at the recruiting clinic are unlikely to benefit from this program as designed.
Why it matters
Potential benefit: If successful, these strategies could make it easier for young people with type 2 diabetes to obtain social supports that improve daily diabetes management and health outcomes.
How similar studies have performed: Similar approaches using community health workers, tailored resources, and text messaging have shown promise in adults and some pediatric settings, but they are less well studied specifically in adolescents with type 2 diabetes.
Eligibility criteria

Adolescent-Caregiver Dyads will be recruited together. Adolescent Inclusion Criteria: * Age 13 to 22 years old * known diagnosis of type 2 diabetes * followed clinically at UPMC Children's Hospital of Pittsburgh * able to provide assent/consent Caregiver Inclusion Criteria: * Adult (18 years or older) identifying as a primary caretaker of an adolescent or young adult with type 2 diabetes * Has an adolescent/young adult who agrees to participate in the study * able to provide consent Exclusion Criteria: * Inability to complete study questionnaires in English
