What this trial studies

This project aims to enhance medication management among older individuals with limited life expectancy through a patient-centered deprescribing intervention. The intervention involves a series of at least three consultations between the patient and their general practitioner (GP) to align medication use with the patient's preferences and goals. By continuously adjusting medications based on the patient's needs, the study seeks to improve overall quality of life for participants. The trial will compare outcomes between those receiving the intervention and those receiving usual care.

Who should consider this trial

Good fit: Ideal candidates for this study are individuals aged 80 years and older who are taking at least eight different medications and have a life expectancy of less than two years.

Not a fit: Patients who are unable to communicate or do not speak and understand Danish may not benefit from this study.

Eligibility criteria

Inclusion Criteria:

* Aged ≥ 80 years
* Take ≥ 8 different medications
* Have a life expectancy of \<2 years, estimated via the Surprise Question ("Would you be surprised if this patient died within 1-2 years?") by the general practitioner
* have a Mini-Mental State Exam (MMSE) score of ≥18
* Are able to provide informed consent

Exclusion Criteria:

* Not able to communicate
* Does not speak and understand Danish
