This study is testing a new way for doctors, nurses, and pharmacists to work together to help seniors with major neurocognitive disorders manage their medications better and stay healthy.

What this trial studies
This project aims to enhance pharmacotherapy for seniors with major neurocognitive disorders by implementing a new care model that promotes collaboration among healthcare professionals, including pharmacists, nurses, and doctors. The intervention involves knowledge exchange sessions and support for seniors to reduce polypharmacy and inappropriate medications while maintaining their cognitive health and autonomy. Approximately 400 seniors will be divided into intervention and control groups to evaluate the effectiveness of the new care approach over a 12-month period, with follow-up assessments conducted for about 6 months.
Who should consider this trial
Good fit: Ideal candidates are seniors aged 65 and older who are undergoing cognitive evaluation or have been diagnosed with major neurocognitive disorders and are taking prescription medications.
Not a fit: Patients in palliative care or those unable to respond to questionnaires in French without a caregiver may not benefit from this study.

Eligibility criteria

Inclusion Criteria: * All seniors (65 years of age or older) undergoing cognitive evaluation OR referred to a memory clinic OR having been diagnosed with cognitive impairment within the last year OR with MCND and followed up at home AND, * referred to the pharmacist, for the FMGs exposed * taking prescription medications Exclusion Criteria: * Seniors in palliative care OR * unable to answer questionnaires in French AND without a caregiver.
