What this trial studies
This study focuses on optimizing drug prescriptions for elderly patients living in nursing homes, particularly those at high risk of adverse drug reactions. It involves the collaboration of clinical pharmacists with mobile geriatric teams to conduct thorough assessments and provide tele-expertise in medication management. The goal is to reduce unplanned hospitalizations by addressing issues related to polypharmacy and potentially inappropriate drugs. The study is set within a university-based framework, emphasizing a comprehensive approach to geriatric care.
Who should consider this trial
Good fit: Ideal candidates for this study are elderly individuals aged 65 and older residing in nursing homes who are at high risk for medication-related issues.
Not a fit: Patients with a life expectancy of less than 3 months or those with severe dementia may not benefit from this study.
Why it matters
Potential benefit: If successful, this approach could significantly reduce the risk of adverse drug events and unplanned hospitalizations among elderly nursing home residents.
How similar studies have performed: Other studies have shown that interventions involving clinical pharmacists can effectively reduce medication-related problems in elderly populations, suggesting a promising approach.
