What this trial studies
This implementation study aims to enhance the management of hypertension and diabetes in rural Bangladesh through integrated, decentralized primary care. It will document the experiences of healthcare provider training, the use of mobile health (mHealth) technologies, and community-based care strategies. The study will compare the effectiveness and cost-effectiveness of this multicomponent approach against usual care and mHealth interventions alone, while also exploring barriers and facilitators to successful implementation. Mixed-methods research will be employed to assess lifestyle changes and patient burden at both individual and community levels.

Inclusion Criteria: * All participants aged 40 or above * With hypertension and diabetes * Residents of specific study areas * Willing to participate Exclusion Criteria: * Adults aged less than 40 years * Children * Pregnant women * Severely ill * Having severe disabilities both physical and mental)
