What this trial studies
This study tests a culturally-sensitive, telephone-based intervention aimed at enhancing the physical and mental health of Veterans diagnosed with heart failure. Participants will engage in a 12-week telehealth program, partnering with a registered nurse for eight telephone sessions. The study will evaluate the feasibility of recruitment, program acceptability, and the effectiveness of the intervention on various health outcomes, including self-care and quality of life. Follow-up assessments will occur at multiple time points to determine the sustainability of the intervention's effects.
Who should consider this trial
Good fit: Ideal candidates for this study are US Veterans aged 18 and older who have been diagnosed with heart failure and have reliable telephone access.
Not a fit: Patients with a history of cognitive dysfunction may not benefit from this study due to the nature of the intervention.

Inclusion Criteria: * Age 18 years of age or older * US Veteran * Diagnosed with heart failure (reduced or preserved ejection fraction) * Able to read, speak, and understand English * Reliable telephone access Exclusion Criteria: • History of cognitive dysfunction
