Improving drug prescriptions for elderly patients to reduce risks
Optimization of Drug Prescriptions in the Care Pathway for the Elderly, With the Final Objective of Reducing the Iatrogenic Risk.

What this trial studies
The IATROPREV project aims to enhance the safety and relevance of drug prescriptions for elderly patients who are hospitalized, particularly those on multiple medications. This observational study focuses on improving coordination of care between hospitals and community healthcare providers through better information sharing and multi-professional consultations. By addressing issues related to iatrogenic effects and drug interactions, the project seeks to optimize treatment pathways for vulnerable elderly patients. The initiative is part of a broader organizational innovation effort in the Hauts-de-France region.
Who should consider this trial
Good fit: Ideal candidates for this study are elderly patients hospitalized in the geriatric medicine department who are on multiple medications and at risk of iatrogenic effects.
Not a fit: Patients who are not hospitalized in the geriatric medicine department will not benefit from this study.
Why it matters
Potential benefit: If successful, this project could significantly reduce the risk of adverse drug events in elderly patients, leading to safer and more effective medication management.
How similar studies have performed: Other studies have shown success in improving medication management and reducing iatrogenic risks in elderly populations, indicating that this approach has potential.
