What this trial studies
This intervention focuses on enhancing digital literacy among older adults to improve their quality of life and health communication. Participants will receive personalized support through home visits and phone calls, tailored to their individual needs and goals. The program includes providing necessary technology, such as tablets and internet connectivity, to facilitate learning. The intervention aims to address barriers identified by the community and is designed using Human-Centered Design techniques.

Good fit: Ideal candidates for this intervention are older adults aged 65 and above who are proficient in English and willing to improve their digital skills.
Not a fit: Patients with severe cognitive impairment or those who have been hospitalized overnight more than three times in the last year may not benefit from this intervention.

Inclusion Criteria: * Aged 65 years old or older * English proficient * Willingness to improve digital literacy Exclusion Criteria: * Severe cognitive impairment as defined by a 6-item Callahan Cognitive Screener * Hospitalization overnight more than 3 times in the last 12 months by self-report.
