What this trial studies
This randomized control trial involves 100 participants who are formerly incarcerated Black men with poorly controlled type 2 diabetes. The study aims to assess the impact of a tailored nurse case management intervention on glycemic control, self-care behaviors, and quality of life over a six-month period. Participants will receive standard diabetes education along with personalized support from trained nurse case managers who will assist with healthcare navigation and basic needs. The intervention is designed to address the unique health challenges faced by this population.
Who should consider this trial
Good fit: Ideal candidates for this study are Black men aged 18 and older who have a clinical diagnosis of type 2 diabetes and have been recently released from incarceration.
Not a fit: Patients who are currently participating in other diabetes clinical trials or have significant mental health issues may not benefit from this study.
Why it matters
Potential benefit: If successful, this approach could significantly improve diabetes management and overall health outcomes for formerly incarcerated Black men.
How similar studies have performed: While similar approaches have been explored, this specific intervention targeting formerly incarcerated Black men with diabetes is relatively novel.
