What this trial studies

This study aims to enhance diabetes management in China by implementing a localized share care model, originally developed in Taiwan. It involves collaboration between diabetic experts, educators, and community doctors using an interactive information platform and mobile applications for self-monitoring. The study will compare the outcomes of patients managed under this new model against those receiving conventional care across six diabetic centers over a five-year period.
