What this trial studies
This project evaluates an intervention aimed at enhancing entry to care for patients recently diagnosed with Type 2 Diabetes Mellitus (T2DM) to alleviate diabetes distress. The intervention focuses on strengthening cross-sectoral collaboration and ensuring patients receive adequate support and referrals for health education and lifestyle management.
