What this trial studies

This project evaluates an intervention aimed at enhancing entry to care for patients recently diagnosed with Type 2 Diabetes Mellitus (T2DM) to alleviate diabetes distress. The intervention focuses on strengthening cross-sectoral collaboration and ensuring patients receive adequate support and referrals for health education and lifestyle management. It is structured over the first three months post-diagnosis, addressing the emotional and informational needs of patients. By improving the organizational structure of diabetes care, the study seeks to reduce the psychological burden associated with living with T2DM.
