This will try doing a two-hour glucose tolerance test in the hospital right after delivery instead of waiting six weeks to see if that finds diabetes earlier in people who had gestational diabetes.

What this trial studies
This single-center randomized protocol assigns people with gestational diabetes to either an in-hospital immediate two-hour oral glucose tolerance test (OGTT) during the postpartum stay or the usual-care OGTT at the six-week postpartum visit. The trial compares diagnosis rates and uptake of postpartum diabetes screening between the two groups and tracks early detection of type 2 diabetes or impaired glucose metabolism. The approach is motivated by high missed-visit rates after delivery and prior data showing immediate postpartum testing yields comparable diagnostic values with much higher adherence. The work is conducted at UMass Memorial Medical Center and targets patients receiving prenatal care there who plan to deliver at that hospital.
Who should consider this trial
Good fit: People whose pregnancy was complicated by gestational diabetes (per Carpenter-Coustan criteria), who receive prenatal care at and plan delivery at UMass Memorial, can complete a two-hour OGTT, and can give informed consent.
Not a fit: People with known pre-gestational diabetes, those unable to complete an OGTT (for example due to prior gastric surgery or vomiting), or those on systemic steroids are not expected to benefit from this approach.
Why it matters
Potential benefit: If successful, this could greatly increase screening uptake and catch postpartum diabetes earlier so patients can start treatment and reduce long-term risk.
How similar studies have performed: Previous evaluations have shown immediate in-hospital postpartum OGTT gives diagnostic results similar to standard postpartum testing and nearly 100% adherence, but broad clinical implementation remains limited.

Inclusion Criteria: * pregnancy complicated by GDM as diagnosed with traditional two step testing in the third trimester according to Carpenter-Coustan Criteria * receiving prenatal care at UMASS Memorial and plans to deliver at UMASS Memorial * able and willing to provide informed consent * ability to complete immediate in hospital postpartum glucose testing * have evidence of impaired glucose metabolism defined as fasting glucose value of ≥126 mg/dL or 2-hour glucose value of ≥200 mg/dL Exclusion Criteria: * known diagnosis of pre-existing pre-gestational diabetes in pregnancy * inability to complete oral glucose tolerance test (e.g. gastric bypass surgery history, gastric dumping syndrome history, vomiting of oral glucose tolerance test) in pregnancy or immediately postpartum * systemic steroid use
