How pulmonary endarterectomy affects confidence in managing chronic thromboembolic pulmonary hypertension
Cardiac Self-Efficacy in Patients With Chronic Thromboembolic Pulmonary Hypertension
Damascus University · NCT07272798
This project will see if pulmonary endarterectomy helps people with chronic thromboembolic pulmonary hypertension feel more confident managing their condition.

What this trial studies
This single-center prospective observational cohort will enroll adults with CTEPH scheduled for elective pulmonary endarterectomy at Damascus University Cardiac Surgery Hospital. Participants will complete validated self-efficacy questionnaires (PEPPI, SEMCD-6, SCSES) before surgery and at predefined postoperative follow-ups while functional capacity is measured using the 6-minute walk test. The analysis will link changes in patient-reported confidence with objective physical outcomes after PEA. Results aim to clarify the psychological course of recovery and inform supportive interventions.
Who should consider this trial
Good fit: Adults with CTEPH who are scheduled for elective pulmonary endarterectomy and can read, consent, and complete questionnaires are ideal candidates.
Not a fit: Patients treated only with balloon pulmonary angioplasty or medical therapy, those undergoing emergency PEA, or individuals unable to complete questionnaires due to severe cognitive or language barriers are unlikely to benefit or be eligible.
Why it matters
Potential benefit: If successful, the findings could lead to targeted support programs that boost patients' confidence and improve recovery and daily functioning after pulmonary endarterectomy.
How similar studies have performed: This approach is novel for CTEPH—previous research has emphasized physiological and hemodynamic outcomes after PEA, and longitudinal data on cardiac self-efficacy in this population are limited.
Eligibility criteria
Show full inclusion / exclusion criteria
Inclusion Criteria: * all patients undergoing pulmonary thromboendarterectomy for CTEPH * Able to read and understand the informed consent form and study questionnaires Exclusion Criteria: * Patients undergoing Balloon Pulmonary Angioplasty (BPA) or receiving only medical therapy (as the focus is on the surgical cohort). * Emergency or salvage PEA procedures (to ensure stable baseline assessment). * Inability to complete the questionnaires due to severe cognitive impairment, psychiatric illness, or language barrier.
