We will test whether different grades of chondromalacia patella change static and dynamic balance in adults aged 18 to 40.

What this trial studies

This observational study will recruit 100 participants including 80 patients with MRI-confirmed chondromalacia patella (20 each for grades I–IV) and 20 healthy controls. Participants will undergo static and dynamic balance testing using the Biodex Balance System at the Faculty of Physical Therapy, Cairo University. Eligible patients are adults 18–40 years old with BMI 19.0–29.0, chronic symptoms for more than three months, and a positive patellar grind test, while those with inflammatory arthritis, vestibular or neurological disorders are excluded.

Who should consider this trial

Good fit: Ideal candidates are adults aged 18–40 with MRI-confirmed unilateral or bilateral chondromalacia (grade I–IV), BMI 19.0–29.0, symptom duration over three months, and a positive patellar grind test.

Not a fit: Patients with rheumatoid or other inflammatory arthritis, vestibular or neurological disorders, recent intra-articular knee injections (within 3 months), prior knee replacement, or those outside the age/BMI ranges are unlikely to benefit from or qualify for this protocol.

Why it matters

Potential benefit: If successful, the results could help tailor rehabilitation and fall-prevention strategies by identifying grade-specific balance deficits in people with chondromalacia.

How similar studies have performed: Previous work has reported balance impairments in people with knee osteoarthritis and patellofemoral pain, but using the Biodex to compare balance across specific chondromalacia grades is relatively novel.

Eligibility criteria

Inclusion Criteria:

1. Age ranged from 18 to 40 years old
2. groups include both males and females
3. Patients with Grade I, II, III and IV chondromalacia: Confirmation of diagnosis by MRI
4. BMI will be ranged from 19.0 to 29.0
5. Patients with Unilateral or bilateral chondromalacia.
6. Chronic cases for more than 3 months.
7. Positive sign of Knee pain according to patellar Grind Test

Exclusion Criteria:

1. patients have not undergone knee replacement.
2. patients have not done Intra-articular injection in the affected knee for the past 3 months.
3. Patients with rheumatoid arthritis or other inflammatory arthritis.
4. Patients with Vestibular disorders or neurological conditions.
