This 12-week home telerehabilitation program will try guided exercise and rehabilitation for men in rural areas who are on androgen-deprivation therapy for high-risk or metastatic prostate cancer to see if it's usable, acceptable, and helps them stick with exercise.

What this trial studies
Men receiving androgen-deprivation therapy for high‑risk or metastatic prostate cancer who live in rural areas will participate in a 12‑week multimodal telerehabilitation program delivered via a telerehabilitation unit. The intervention combines remotely supervised exercise, functional rehabilitation, and patient education delivered through telemedicine. The pilot will measure usability, acceptability, and exercise adherence over the 12 weeks and record barriers and facilitators to participation. Results will inform feasibility and design of larger trials to improve access to cancer rehabilitation in rural communities.
Who should consider this trial
Good fit: Men aged 18 or older with confirmed prostate cancer who are receiving standard ADT for high‑risk, locally advanced, or metastatic disease and who live in a rural RUCA zip code, without unstable cardiac or painful/unstable bony metastases, are ideal candidates.
Not a fit: Patients with unstable angina, uncontrolled hypertension, recent myocardial infarction, pacemakers, painful or unstable bone metastases, recent fractures, those already in a regular exercise rehabilitation program, or those unable to use telehealth are unlikely to receive benefit from this intervention.
Why it matters
Potential benefit: If successful, the program could expand access to rehabilitation for rural patients, improve physical function and quality of life, and increase adherence to exercise during ADT.
How similar studies have performed: Previous pilot work showed high acceptance of home telerehabilitation in older adults and positive feasibility signals in advanced bladder cancer, but systematic testing in rural advanced prostate cancer is novel.

Inclusion Criteria: * Age 18 and older * Confirmed diagnosis of prostate cancer * Men receiving standard-of-care ADT either for high-risk, locally advanced prostate cancer or as a part of multicomponent management of metastatic prostate cancer * Residing in a rural community, as indicated by zip code in RUCA areas Exclusion Criteria: * Unstable angina, uncontrolled hypertension, recent myocardial infarction, pacemakers, painful or unstable bony metastases, or recent skeletal fractures * Engaged in a regular exercise rehabilitation program * Relocation plans within next 3 months * Participating in another clinical trial * Have a working telephone line in their home or a cell phone.
