What this trial studies
This pilot study evaluates a passive sensor network developed by Sensorum Health to identify early signs of health decompensation in elderly patients with a history of congestive heart failure (CHF). The goal is to detect these signals before a hospitalization occurs, allowing for timely interventions. Participants will be monitored to collect data that could inform future interventional studies aimed at improving patient outcomes. The study focuses on patients aged 55 and older who have experienced recent hospital utilization due to their condition.
Who should consider this trial
Good fit: Ideal candidates are patients aged 55 and older with a documented diagnosis of CHF who have had recent hospital utilization.
Not a fit: Patients with significant cardiac valvular disease, end-stage renal disease, end-stage CHF, or end-stage COPD may not benefit from this study.

Inclusion Criteria: * Current Patient at Weill Cornell Medicine * Aged 55 years or older * Able to consent * Documented diagnosis of congestive heart failure (CHF) * At least 1 of the following prior hospital utilization events in the past 12 months * Inpatient admission for any reason * Facility observation stay for any reason * Emergency Department visit for any reason Exclusion Criteria: * Significant cardiac valvular disease * End-Stage Renal Disease (ESRD) * End-Stage CHF * End-Stage COPD
