This will test whether taking 500 mg of hesperidin daily for 12 weeks helps reduce fatigue and improve thinking and mood in adults with multiple sclerosis.

What this trial studies
This is a remote, randomized, double-blind, placebo-controlled, parallel-group trial that assigns adults with MS to 500 mg/day hesperidin or a matching placebo for 12 weeks. The primary outcome is change in fatigue (Modified Fatigue Impact Scale) with secondary outcomes including cognitive performance and mood measured at baseline and week 12. Data will be analysed using linear mixed-effects models comparing change from baseline with fixed effects for group, time, and the group × time interaction and a participant-level random intercept. The rationale is based on preclinical EAE models suggesting hesperidin has anti-inflammatory and antioxidant effects, but human randomized data are limited.
Who should consider this trial
Good fit: Adults (age ≥18) with a clinical diagnosis of multiple sclerosis who do not have significant gastrointestinal problems or known food/citrus allergies are the intended participants.
Not a fit: People under 18, those with known food allergies or major GI issues, or patients whose symptoms are driven by other medical conditions are unlikely to benefit from this intervention.
Why it matters
Potential benefit: If effective, hesperidin could provide an affordable, low-risk supplement option to reduce fatigue and improve cognition and mood in people with MS.
How similar studies have performed: Animal (EAE) studies show promising anti-inflammatory and neuroprotective effects of hesperidin, but randomized controlled trials in people with MS are scarce.

Inclusion Criteria: * Diagnosis of Multiple Sclerosis, Aged 18 years or older. Exclusion Criteria: * Known food allergies, intolerances, or significant gastrointestinal (GI) problems, Participants under the age of 18 years.
