This study is testing ways to help teenagers with metabolic disorders smoothly move to adult healthcare by improving their knowledge and skills, and seeing if these changes make a difference in their lives over time.

What this trial studies
This project aims to evaluate the current transition process for adolescents with inborn errors of metabolism as they prepare to move to adult healthcare services. It will assess their knowledge, skills, and quality of life, and provide structured interventions to improve their readiness for this transition. The effectiveness of these interventions will be measured shortly after implementation and again at 6 and 12 months, focusing on both psychological and biochemical outcomes.
Who should consider this trial
Good fit: Ideal candidates are adolescents aged 14 and older with an inborn error of metabolism who are currently receiving care from a specialized metabolic unit.
Not a fit: Patients who do not speak German, have cognitive impairments, are in end-of-life situations, or have diseases without established biochemical or physical markers may not benefit from this study.

Eligibility criteria

Inclusion Criteria:Patients * with an inborn error of metabolism in the care of a specialized metabolic care unit * requiring specialized adult metabolic care * at least 14 years old * with a disease for which at least one biochemical and / or physical parameter disease marker is well established . Exclusion Criteria: Patients - * with insufficient knowledge of the German * with cognitive impairment to a degree that consent, and participation would be impossible * in an end-of-life situation * with a disease for which no biochemical and / or physical parameter disease marker is well established
