This program will test whether giving adults in Anson County healthy prepared meals and nutrition education helps them eat better than giving them physical activity education and coaching.

What this trial studies

The Healthy Living Anson program randomizes English-speaking adults (18+) who live in Anson County and have or are at risk for chronic disease to receive either a Food-is-Medicine intervention (healthy frozen meals or a produce box plus nutrition education from a Community Health Advocate) or a physical activity education and coaching program for four months. After four months the groups swap so that each participant receives both interventions over an eight-month period. Data collection occurs at baseline, midpoint, and study end and includes a funder-required survey (demographics, social, behavioral) plus study-specific questions and in-person biometrics, with surveys completed online or in person as needed. The primary focus is on change in diet quality using a randomized delayed-intervention control design.

Who should consider this trial

Good fit: Ideal participants are English-speaking Anson County residents aged 18 or older who have or are at risk for a chronic disease, can store frozen meals, and can travel to a central location for data collection and classes.

Not a fit: People with dietary restrictions or food allergies incompatible with a Mediterranean-style diet, those who cannot store frozen meals or travel to the study site, and non-English speakers are unlikely to be eligible or to benefit.

Potential benefit: If successful, the program could help participants improve diet quality and related health measures such as weight, blood pressure, and blood sugar.

How similar studies have performed: Previous Food-is-Medicine and medically tailored meal programs have shown improvements in diet quality and some cardiometabolic outcomes, so this approach has supporting evidence.

Inclusion Criteria:

* Potential participants must be a resident of Anson County.
* At least 18 years of age.
* Have or be at risk for a chronic disease.
* Ability to make own dietary decisions.
* Ability to travel to a central Anson County location for data collection and group class sessions.
* Ability and willingness to try the prepared meals.
* Ability to store 10 frozen meals in a freezer.
* Ability to communicate in English
* No dietary restrictions or food allergies such that a Mediterranean diet is not appropriate.
