FAPi PET/CT scan for pancreatic ductal adenocarcinoma

FAP-1 Study: FAPi PET in Pancreatic Ductal Adenocarcinoma: A Prospective, Exploratory Study

Observational Tata Memorial Hospital · NCT07342231

This test uses a Gallium-68–labeled FAP inhibitor with PET/CT to see if it can detect tumor tissue and tumor-associated stroma in adults with biopsy-proven pancreatic ductal adenocarcinoma.

What this trial studies

This is an observational imaging protocol using Ga-68 FAPi PET/CT to visualize fibroblast activation protein (FAP) in and around pancreatic ductal adenocarcinoma. The tracer (a Gallium-68–labeled FAP inhibitor peptide) binds to FAP expressed in the tumor microenvironment and PET/CT images are acquired to map tracer uptake. Patients enrolled are adults with biopsy-proven pancreatic adenocarcinoma and without concurrent malignancy, and pregnant or lactating patients are excluded. Images will be compared to standard imaging to characterize additional tissue detection beyond conventional CT.

Who should consider this trial

Good fit: Adults over 18 with biopsy-proven pancreatic ductal adenocarcinoma who are willing to undergo Ga-68 FAPi PET/CT and have no other active cancers are ideal candidates.

Not a fit: Patients with concurrent malignancies, pregnant or lactating individuals, or tumors that do not express FAP may not receive added benefit from this imaging approach.

Why it matters

Potential benefit: If successful, this approach could help clinicians find more tumor and tumor-associated tissue than standard CT, potentially improving staging and treatment planning.

How similar studies have performed: Early clinical reports across several tumor types, including pancreatic cancer, have shown promising tumor visualization with FAPi PET, though large definitive outcome trials are limited.
