What this trial studies
This study evaluates the Nurse Education and Support Team (NEST) Program aimed at improving postpartum care for women in five rural counties in North Carolina. It is a randomized control trial comparing the NEST-Rural care model with usual postpartum care. The program focuses on coordinated care addressing social, mental, and physical health needs, with features like remote blood pressure monitoring and multiple nurse encounters. The goal is to reduce hospital readmissions and emergency department visits while improving adherence to preventive care guidelines.
