What this trial studies
This clinical trial aims to assess the effectiveness and cost-effectiveness of a Virtual Community of Practice (VCoP) delivered through a web-based application compared to traditional self-administered online education for middle-aged individuals with multiple chronic diseases. Participants aged 30 to 60 with at least two chronic conditions will be randomly assigned to either the VCoP intervention or an active control group receiving individual education. The trial will follow participants for 18 months to measure improvements in their health activation and engagement. The study will take place in primary health centers and hospitals across Catalonia, Madrid, and the Canary Islands.
Who should consider this trial
Good fit: Ideal candidates are middle-aged individuals aged 30-60 with at least two chronic diseases.
Not a fit: Patients with terminal illnesses or significant disabilities that hinder their ability to participate may not benefit from this study.
Why it matters
Potential benefit: If successful, this approach could enhance the management and empowerment of patients with chronic diseases, leading to better health outcomes.
How similar studies have performed: Other studies have shown promise in using virtual communities for chronic disease management, suggesting potential success for this approach.
