What this trial studies
This clinical trial aims to assess the effectiveness of a medication-taking habit worksheet combined with pharmacist discussions for patients starting new chronic medications for cardiovascular disease or depression. The study will compare the outcomes of patients receiving this habit-based intervention against those receiving standard pharmacist counseling. Participants will engage in a sequential design, first evaluating a group without the worksheet and then a group with it, while completing pre and post surveys to measure changes in medication adherence beliefs and behaviors. The pharmacist will guide patients in creating personalized cues to help establish their medication-taking habits.

Not a fit: Patients who do not manage their own medications independently will not benefit from this study.

Inclusion Criteria: * Participants must be fluent to speak and write in English, * must be prescribed a new chronic medication Exclusion Criteria: * Does not manage their own medications independently
