What this trial studies

This is a Phase 2, single-site interventional trial testing the COX-2 inhibitor etoricoxib alongside L-dopa in adults diagnosed with Parkinson's disease. The study will enroll patients meeting clinical Parkinson's criteria (using the UPDRS) and excludes pregnant or breastfeeding people, those with significant liver or kidney problems, current users of other anti-inflammatory drugs, and people with substance abuse or allergies to the study medications. Outcomes likely include changes in inflammatory biomarkers such as HMGB1 and receptor pathways (TLR4/RAGE) and clinical Parkinson's measures. The trial is being conducted at Mansoura University in Al Mansurah, Egypt.

Who should consider this trial

Good fit: Adults (age 18 or older) with a clinical diagnosis of Parkinson's disease by UPDRS who are not pregnant or breastfeeding and who do not have significant liver or kidney impairment may be eligible.

Not a fit: People who are pregnant or breastfeeding, have significant liver or kidney dysfunction, are using other anti-inflammatory drugs, have drug allergies to the study medications, or have active substance abuse are unlikely to qualify or benefit from participation.

Why it matters

Potential benefit: If successful, the treatment could reduce neuroinflammation and potentially improve symptoms or slow progression in some people with Parkinson's disease.

How similar studies have performed: Preclinical work and small clinical studies of COX-2 inhibitors suggest possible anti-inflammatory effects, but clear clinical benefit in Parkinson's disease remains unproven.

Inclusion Criteria:

* Age ≥ 18 years.
* Both male and female will be included.
* Patients diagnosed with PD according to Unified Parkinson's Disease Rating Scale.

Exclusion Criteria:

* Breast feeding
* Patients with significant liver and kidney function abnormalities.
* Alcohol and / or drug abusers.
* Patients with known allergy to the study medications
* Pregnant women and women with planned pregnancy.
* Patients who are currently using other anti-inflammatory drugs.
