This trial will test whether an erector spinae plane block reduces pain compared with standard multimodal analgesia in adults aged 21–60 undergoing ESWL for 2–3 cm kidney stones.

What this trial studies

This interventional study compares ultrasound-guided erector spinae plane block (ESPB) with conventional multimodal analgesia including nalbuphine for adults undergoing extracorporeal shock wave lithotripsy (ESWL) for 2–3 cm renal stones. Eligible participants aged 21–60 with BMI 18–30 and ASA I–II at Sohag University Hospital will be assigned to receive either ESPB before ESWL or standard systemic analgesics. Pain scores during and after the procedure, total analgesic consumption, and any complications or side effects will be recorded. The goal is to see if ESPB provides better pain control and lowers opioid use during and after ESWL.

Who should consider this trial

Good fit: Adults aged 21–60 with ASA physical status I–II undergoing ESWL for 2–3 cm renal stones, BMI 18–30 kg/m², and able to give informed consent are ideal candidates.

Not a fit: Patients with coagulopathy or on anticoagulants, infection at the injection site, BMI ≥30, advanced organ or psychiatric disease, pregnancy or breastfeeding, or allergies to study drugs are unlikely to be eligible or to benefit.

Why it matters

Potential benefit: If successful, the block could provide better pain relief during ESWL and reduce the need for systemic opioids, improving patient comfort.

How similar studies have performed: ESPB has shown promising pain relief in other thoracic and abdominal procedures, but evidence specifically for ESWL is limited.

Inclusion Criteria:

* Adult Patients undergoing ESWL with 2-3 cm renal stones
* patients between the ages of 21 and 60,
* both sexes,
* BMI ≥18≤30 kg/m²
* Able to give informed consent.
* Patients older than 18 years with American Society of Anesthesiologists Physical Status I or II and

Exclusion Criteria:

* Coagulopathy or on anticoagulants
* Infection at the ESPB injection site
* BMI ≥30 kg/m²
* Patient refusal.
* patients with advanced respiratory, renal, hepatic, neurological, or psychiatric disease, pregnant or nursing women.
* patients with a history of allergies to any study drugs
