This study will test whether endoscopic duct procedures and/or shockwave lithotripsy help adults with painless chronic pancreatitis who have not had these treatments before improve pancreatic function and related symptoms.

What this trial studies
This interventional study enrolls adults aged 18–85 with painless chronic pancreatitis and no prior ERCP or ESWL to receive standard medical care plus endoscopic retrograde cholangiopancreatography (ERCP) and/or extracorporeal shockwave lithotripsy (ESWL) as indicated. Participants may also receive lifestyle advice, pancreatic enzyme replacement, and antidiabetic medications according to clinical need. Key exclusions include complications requiring surgical intervention, autoimmune pancreatitis, suspected malignancy, prior pancreatic or gastrointestinal bypass surgery, end-stage disease, pregnancy, and contraindications to ERCP/ESWL. The study is conducted at Changhai Hospital in Shanghai and compares clinical outcomes following these interventions in the painless CP population.
Who should consider this trial
Good fit: Adults 18–85 diagnosed with painless chronic pancreatitis who have not previously undergone ERCP or ESWL and who do not have surgical complications or contraindications are ideal candidates.
Not a fit: Patients with complications that require surgery, autoimmune pancreatitis, suspected cancer, prior pancreatic surgery, end-stage disease, pregnancy, or contraindications to ERCP/ESWL are unlikely to benefit from enrollment.
Why it matters
Potential benefit: If successful, the treatments could improve pancreatic duct clearance and help preserve or improve endocrine and exocrine pancreatic function in people with painless chronic pancreatitis.
How similar studies have performed: ERCP and ESWL have shown benefit for stone clearance and pain relief in obstructive or painful chronic pancreatitis, but their specific benefit in the painless chronic pancreatitis subgroup is less well studied.

Inclusion Criteria: * Adults aged 18-85 years. * Diagnosed with painless chronic pancreatitis. * No prior history of endoscopic retrograde cholangiopancreatography (ERCP) or extracorporeal shockwave lithotripsy (ESWL) treatment. Exclusion Criteria: * Presence of complications requiring endoscopic or surgical intervention, such as pancreatic pseudocysts, benign biliary strictures, or pancreatic fistulas. * Autoimmune pancreatitis. * Suspected malignancy. * History of pancreatic surgery or gastrointestinal bypass surgery (e.g., Billroth II procedure). * End-stage disease. * Pregnancy or lactation. * Contraindications to ESWL or ERCP. * Refusal to provide informed consent.
