What this trial studies

This is a proof-of-concept randomized controlled trial recruiting 100 pregnant women with BMI ≥23 in their first trimester at KK Women’s and Children’s Hospital. Participants will be block-randomized (block size 2) to receive the e-IMPACT integrated eHealth programme or usual care and will be followed through the second and third trimesters until delivery. The intervention offers personalised guidance, credible information, and improved communication with healthcare professionals to support adherence to Singapore’s physical activity guidelines in pregnancy. Outcomes will focus on changes in physical activity, self-efficacy, and related maternal weight measures.

Who should consider this trial

Good fit: Pregnant women aged 21 or older in their first trimester with singleton pregnancies, BMI ≥23 kg/m², physically able to do moderate exercise, and able to understand English are ideal candidates.

Not a fit: Women with severe chronic diseases, obstetric or medical complications that limit normal activity, recent participation in other weight-management or activity interventions, or who cannot understand English are unlikely to benefit from this programme.

Why it matters

Potential benefit: If successful, the programme could increase the number of pregnant women meeting physical activity guidelines and reduce excessive gestational weight gain.

How similar studies have performed: Digital and behavioural interventions in pregnancy have shown modest improvements in activity and weight-related outcomes, but integrated, culturally tailored eHealth programmes for overweight pregnant women in Singapore are relatively untested.

Eligibility criteria

Inclusion Criteria:

1. Pregnant women (First trimester at least 4 weeks gestation)
2. Age 21 years and above
3. BMI 23 kg/m2 and above
4. Physically fit to participate in moderate intensity exercise
5. Singleton pregnancy

Exclusion Criteria:

1. Women who have severe chronic disease such as chronic kidney disease, hypertension, asthma OR
2. Obstetric or medical complications which might preclude women in normal levels of physical activity or exercise during pregnancy.
3. Women who participated in other weight management programmes, or physical activity intervention studies recently (past 6 months) or concurrently
4. Difficulty in understanding the English language
