This study will see if starting cardiopulmonary rehabilitation early during a hospital stay helps people with acute decompensated heart failure regain breathing and physical function and leave the hospital sooner.

What this trial studies

This interventional study compares early, exercise-based cardiopulmonary rehabilitation started during hospitalization with routine in-hospital care for patients admitted with acute decompensated heart failure who are clinically stable. Participants receive a multidisciplinary rehabilitation program targeting physical and respiratory function in addition to standard medical treatment, while the control group receives routine care alone. Outcomes of interest include physical performance, respiratory parameters, oxygen dependence, and clinical endpoints such as length of stay. The trial excludes patients with major non-cardiac comorbidities, recent heart surgery, thromboembolic events, or significant lower-extremity wounds.

Who should consider this trial

Good fit: Ideal candidates are patients hospitalized with acute decompensated heart failure who are clinically stable and do not have major pulmonary, neurological, renal, hepatic, gastrointestinal, orthopedic, or oncological conditions, cardiomyopathy or constrictive pericarditis, recent heart surgery, thromboembolism, or significant lower-extremity wounds.

Not a fit: Patients with the listed exclusions—significant noncardiac comorbidities, cardiomyopathy or constrictive pericarditis, recent heart surgery, active or suspected thromboembolism, or open wounds on the lower extremities—are not eligible and are unlikely to benefit from this intervention within the trial.

Why it matters

Potential benefit: If successful, early cardiopulmonary rehabilitation could speed recovery of breathing and physical function, reduce oxygen dependence, and shorten hospital stays for patients with acute decompensated heart failure.

How similar studies have performed: Prior work in chronic heart failure populations suggests early cardiopulmonary rehabilitation can improve physical function and some cardiac measures, but evidence specifically in acute decompensated heart failure is limited and mixed.

Inclusion Criteria:

* Patients hospitalized with a diagnosis of acute decompensated heart failure
* Being clinically stable

Exclusion Criteria:

* Having diagnosed pulmonary, neurological, renal, liver, gastrointestinal, orthopedic or oncological pathologies
* Having cardiomyopathy or congestive pericarditis
* Recent heart surgery in the past 6 months
* New or suspected thromboembolic event
* Presence of open wounds, ulcerations or major dermatological diseases in the lower extremities
