Doppler ultrasound to predict success of conservative or methotrexate treatment for tubal ectopic pregnancy

Doppler Flow in Tubal Ectopic Pregnancy in Diagnosis as a Predictor of Treatment Success - Conservative and MTX Treatment

Meir Medical Center · NCT07541937

This project will test whether specific Doppler blood-flow measurements at diagnosis can help predict if conservative management or methotrexate will successfully treat tubal ectopic pregnancy.

What this trial studies

This is a prospective study enrolling about 60 women with hemodynamically stable tubal ectopic pregnancies at diagnosis. Investigators will record Doppler ultrasound parameters (resistive index, pulsatility index, and peak systolic velocity) and clinical/laboratory data, then follow patients treated according to standard departmental protocols (conservative monitoring or methotrexate). The primary aim is to see whether Doppler measures at presentation predict treatment success; secondary aims include exploring other factors such as gestational features, pain, bleeding, and beta-hCG levels. Results could clarify whether Doppler vascularization should inform selection of non-surgical management.

Who should consider this trial

Good fit: Women aged 18–45 with a confirmed tubal ectopic pregnancy who are hemodynamically stable and eligible for conservative management or methotrexate therapy are ideal candidates.

Not a fit: Patients who are hemodynamically unstable, have suspected tubal rupture, non-tubal ectopic pregnancies, or require immediate surgery are unlikely to benefit from this approach.

Why it matters

Potential benefit: If successful, clinicians could more accurately select patients for conservative or methotrexate therapy, potentially reducing failed medical treatment and the need for surgery.

How similar studies have performed: Previous smaller studies have suggested a link between Doppler vascularization and treatment outcomes, but findings are mixed and there is no widely accepted threshold yet.
