What this trial studies

A community-health-academic partnership developed Disfrutando, a culturally tailored, bundled program led by community health workers that combines education on healthy eating and physical activity with connections to low- or no-cost social and medical resources. The investigators will run a one-year pilot using a two-group randomized, wait-list controlled design to measure both clinical outcomes (A1C, body weight) and implementation outcomes (acceptability and quality). The intervention is delivered in community settings with a focus on meeting participants where they are and addressing social determinants of health that impede diabetes self-management. The study will also measure changes in self-efficacy and engagement with primary/diabetes/eye care.

Who should consider this trial

Good fit: Adults with elevated hemoglobin A1C (≥5.7) from high-burden communities who can attend community sessions in the study locations are the intended participants.

Not a fit: Individuals with normal A1C (≤5.6) or those unable to participate in community-based sessions are unlikely to benefit from this intervention.

Why it matters

Potential benefit: If successful, Disfrutando could lower blood sugar, improve self-care skills, and reduce barriers from social needs that make healthy behaviors harder.

How similar studies have performed: Community health worker–led, culturally tailored programs have shown promise in similar populations, and a prior non-controlled feasibility of Disfrutando lowered A1C for 66% of 31 participants.

* Hemoglobin A1C of 5.7 or higher

Exclusion Criteria:

* Hemoglobin A1C of 5.6 or lower
