What this trial studies
The STEPSTONES project in Sweden aims to evaluate a person-centered transition program for adolescents with chronic conditions, specifically asthma and those who have undergone solid organ transplants. This innovative digital adaptation, Digi-STEPSTONES, seeks to improve accessibility and continuity of care by delivering the transition program remotely. The study will compare the effectiveness of this digital approach to traditional in-person care, focusing on outcomes such as patient empowerment, transition readiness, and quality of life. Additionally, it will explore the implementation and scalability of this digital model across various chronic conditions.
