This study will test whether digitally guided immediate implant placement with same-day loading works well for people aged 65 and older who need a tooth removed.

What this trial studies

This is a prospective observational study of digitally assisted immediate implant placement with immediate loading in geriatric patients. A fully digital workflow using CBCT, intraoral scanning, and guided surgery will be used to place implants at the time of extraction and restore them promptly. Outcomes include implant survival, marginal bone level changes, mechanical complications, primary stability progression (insertion torque and ISQ), and patient-reported pain, function, and esthetic satisfaction measured by visual analog scales. Participants will be followed for 12 months to determine the predictability and effectiveness of this approach in the elderly.

Who should consider this trial

Good fit: Ideal candidates are adults aged 65 or older who require tooth extraction with adequate residual bone and can achieve primary stability (insertion torque ≥35 Ncm and/or ISQ ≥60) with ASA physical status I–III and good oral hygiene.

Not a fit: Patients with uncontrolled systemic disease, active untreated periodontal disease, heavy smoking (>10 cigarettes/day), prior or current bisphosphonate therapy, severe bruxism, or inadequate bone/primary stability are unlikely to benefit.

Why it matters

Potential benefit: If successful, this approach could shorten treatment time, reduce the number of surgeries, and speed functional and esthetic recovery for older adults needing implants.

How similar studies have performed: Prior studies have reported favorable outcomes for immediate implant placement and immediate loading in selected patients, but data specifically on fully digital workflows in the geriatric population are limited.

Eligibility criteria

Inclusion Criteria:

* Patients aged ≥65 years
* Indication for tooth extraction with immediate implant placement
* Sufficient residual bone volume to achieve primary implant stability
* Ability to achieve insertion torque ≥35 Ncm and/or implant stability quotient (ISQ) ≥60
* American Society of Anesthesiologists (ASA) physical status I-III
* Adequate oral hygiene and patient compliance

Exclusion Criteria:

* Uncontrolled systemic diseases (e.g., uncontrolled diabetes mellitus)
* History of head and neck radiotherapy
* Active untreated periodontal disease
* Heavy smoking (\>10 cigarettes/day)
* Current or previous bisphosphonate therapy
* Parafunctional habits (e.g., severe bruxism)
