What this trial studies
This clinical trial aims to evaluate the effectiveness of digital literacy training for Latino patients with type II diabetes during their hospital admission. Participants will receive training from a digital navigator focused on using patient portals and telehealth services. The study will compare the outcomes of those who receive this training to those who receive standard educational materials to determine the impact on their use of digital health tools after discharge. The goal is to address disparities in digital health tool usage among Latino patients.

Inclusion Criteria: * Self-identified Latino adult patients with type II diabetes * Admitted to general medicine floors at Brigham and Women's Hospital * Preferred language: English or Spanish Exclusion Criteria: * Critical illness * dementia, delirium, or other condition that limits ability to participate in education and consent process
