What this trial studies
This study evaluates the feasibility and effects of the Diabetes LIVE JustICE application, designed for individuals with diabetes who have recently been released from prison. It employs a randomized clinical trial design with a non-equivalent control group and includes repeated measures at 0, 6, and 12 weeks to assess various outcomes such as diabetes knowledge, self-care behaviors, and clinical measures like glycemic control. Focus groups will also be conducted to gather feedback on the acceptability of the intervention among participants. The goal is to improve diabetes self-management skills in a vulnerable population that often lacks critical health-related knowledge.
Who should consider this trial
Good fit: Ideal candidates for this study are individuals aged 18 and older with Type 1 or Type 2 diabetes who have recently been released from prison.
Not a fit: Patients who do not have diabetes or are not recently released from incarceration may not benefit from this study.

Inclusion Criteria: Individuals will be eligible to participate if they: 1. have Type 1 or 2 diabetes 2. are age 18 and older 3. can speak and understand English 4. have been released directly from one of five Connecticut prisons to supervised community housing or parole
