What this trial studies

This intervention aims to evaluate the effectiveness of a nurse case-manager delivering diabetes self-management education and skills training via telephone to older African Americans with poorly controlled type 2 diabetes. The study will randomly assign 100 participants aged 50 and older to either the DM Social Needs intervention group or a usual care group. The primary focus is on improving clinical outcomes and patient-reported outcomes for those with uncontrolled diabetes. The intervention also addresses unmet social needs that may impact diabetes management.

Who should consider this trial

Good fit: Ideal candidates are African American individuals aged 50 and older with poorly controlled type 2 diabetes (HbA1C >= 8%).

Not a fit: Patients who are actively participating in other diabetes clinical trials or have significant mental confusion or life expectancy concerns may not benefit from this study.

Why it matters

Potential benefit: If successful, this intervention could significantly improve diabetes management and overall health outcomes for older African Americans with type 2 diabetes.

How similar studies have performed: Other studies have shown promise in using community-based interventions for diabetes management, suggesting potential success for this approach.
