What this trial studies
This intervention aims to evaluate the effectiveness of a nurse case-manager delivering diabetes self-management education and skills training via telephone to older African Americans with poorly controlled type 2 diabetes. The study will randomly assign 100 participants aged 50 and older to either the DM Social Needs intervention group or a usual care group. The primary focus is on improving clinical outcomes and patient-reported outcomes for those with uncontrolled diabetes. The intervention also addresses unmet social needs that may impact diabetes management.

Good fit: Ideal candidates are African American individuals aged 50 and older with poorly controlled type 2 diabetes (HbA1C >= 8%).
Not a fit: Patients who are actively participating in other diabetes clinical trials or have significant mental confusion or life expectancy concerns may not benefit from this study.

Eligibility criteria

Inclusion Criteria: 1. Age 50 and older; 2. self-identified as African American or Black; 3. self-reported diagnosis of T2DM; 4. HbA1C \>=8% at the screening visit. Exclusion Criteria: 1. self-reported participation in other diabetes clinical trials; 2. alcohol or drug abuse or dependency as assessed by the CAGE-AID; 3. Mental confusion at screening assessment suggesting significant dementia; 4. life expectancy \< 6 months at screening assessment.
