This project will test detailed invasive coronary physiology and vasospasm tests in adults who have angina but no major coronary blockages to see if microvascular dysfunction or spasms explain their symptoms.

What this trial studies
This observational study enrolls adults with angina referred for coronary angiography who undergo invasive microcirculatory testing and optional acetylcholine provocation for vasospasm. Investigators will measure indices such as CFR, IMR, MRR, and document ECG and symptom responses to classify patients into microvascular angina, vasospastic angina, or non-coronary chest pain endotypes. Data are collected at University Hospital Motol and collaborating Czech centers to define physiological patterns and clinical correlates. The protocol uses measurements obtained during standard catheterization without introducing investigational therapeutic interventions.
Who should consider this trial
Good fit: Adults aged 18 or older with angina or angina-equivalent symptoms who are referred for coronary angiography and receive invasive microcirculation testing with or without vasospasm (acetylcholine) testing.
Not a fit: Patients with clearly obstructive coronary artery disease, those who do not undergo invasive physiology measurements, pregnant or nursing individuals, and people under 18 are unlikely to benefit from this protocol.
Why it matters
Potential benefit: If successful, the study could identify the underlying cause of angina in people without obstructive disease, allowing more targeted treatment and potentially fewer repeat hospital visits.
How similar studies have performed: Previous observational studies and randomized efforts (for example CorMicA) have shown that invasive coronary physiology and vasospasm testing can improve diagnostic precision and, when used to guide therapy, can improve symptoms.

Inclusion Criteria: * Adults of both sexes older than 18 years * Angina symptoms or angina equivalent * Referred to cath lab for evaluation of CAD * Invasive physiology testing performed (microcirculation testing +/- vasospasm testing) Exclusion Criteria: * Persons under the age of 18 * Pregnant of nursing * No coronary physiology measurements were performed
