This project will test whether taking photos of everything you eat (and optionally weighing food for three days) gives accurate calcium and phosphorus balance measurements for adults with stage 4–5 CKD who are not yet on dialysis.

What this trial studies

Adults with CKD stage 4–5 will be asked to photograph all food and drink consumed over 24 hours and offered the option to weigh foods and fluids for three days. Dietary intake from photos and weighed records will be compared with urinary excretion to calculate calcium and phosphorus balance relative to creatinine clearance. The study compares image-based dietary assessment methods with weighed records in a pre-dialysis CKD cohort. It is an observational substudy carried out at Herlev Hospital as part of the CPH-MBD cohort.

Who should consider this trial

Good fit: Adults aged 18 or older with CKD stage 4 or 5 (estimated GFR <30 mL/min/1.73 m2) who are not yet on dialysis.

Not a fit: People on dialysis, those with milder CKD (eGFR ≥30 mL/min/1.73 m2), or anyone unable to reliably photograph or weigh their food are unlikely to benefit.

Why it matters

Potential benefit: If successful, simpler photo-based diet tracking could make it easier to monitor and manage calcium and phosphorus balance in people with advanced CKD.

How similar studies have performed: Photo-based dietary assessment has shown promise in other populations while weighed food records are a common reference standard, but their comparison has been less studied specifically in advanced CKD.

* A glomerular filtration rate of \<30 ml ml/min/1.73m2
* 18 years and above
