What this trial studies
This observational study follows adults over 55 with type 2 diabetes who have been on either SGLT2 inhibitor + metformin, DPP-4 inhibitor + metformin, or a control regimen for at least one year. Researchers will compare cognitive test scores and standard depression questionnaires between the medication groups and review medical records for relevant health data and vascular risk factors. Key exclusions include type 1 diabetes, insulin therapy, end‑organ failure, prior Alzheimer’s disease or major brain injury, and pregnancy or lactation. Who should consider this trial
Good fit: Ideal candidates are adults aged over 55 with type 2 diabetes who have been taking either SGLT2 inhibitor + metformin or DPP-4 inhibitor + metformin for at least one year and can attend Beni Suef Hospital.
Not a fit: People with type 1 diabetes, those on insulin, with end-stage renal, liver, or heart failure, prior Alzheimer’s disease or significant brain injury, or who are pregnant or breastfeeding are unlikely to be eligible or receive benefit from participation.

Inclusion Criteria: \- The study participants should meet the following inclusion criteria: 1. T2DM adult subjects (\>55 years old) will be eligible to participate. 2. Patients should be taking SGLT2Is+metformin or DDP4Is + metformin for at least 1 year to be eligible. Exclusion Criteria: 1- Those with type1 diabetes or ketoacidosis, insulin therapy, end organ failure as chronic renal failure, liver, and heart failure, previous history of pancreatitis, history of Alzheimer's disease or any brain insult, and finally pregnant or lactating females will be excluded from the study.
