What this trial studies

This clinical trial aims to compare the effectiveness of an epidural combined with a bilateral ultrasound-guided transversus abdominis plane (TAP) block versus an epidural alone in patients undergoing elective major laparotomy for gynecologic procedures. Participants will be randomized into two groups: one receiving the combined treatment and the other receiving only the epidural. Pain scores and the requirement for epidural infusion will be assessed postoperatively by a blinded acute pain service team. The study will involve approximately 46 subjects and will last about two days for each participant.

Inclusion Criteria:

* Elective major laparotomy gynaecology procedure.
* Age 18 years and above
* ASA I, II, III

Exclusion Criteria:

* Prolonged INR
* Allergic to LA
* History of chronic pain
* Psychological disorder/addict to opioid or benzodiazepine
* Any contraindication for epidural or TAP block procedure
* Pregnancy
