What this trial studies

This observational study evaluates the diagnostic accuracy of cardiovascular magnetic resonance (CMR) and computed tomography with fractional flow reserve (CT-FFR) in patients suspected of having coronary artery disease (CAD). It aims to determine how well these imaging techniques perform compared to invasive fractional flow reserve (FFR), which serves as the reference standard. The study will involve 300 patients referred for invasive coronary angiography, with a subset undergoing additional CMR scans for comparison. The goal is to improve the specificity and positive predictive value of CAD diagnosis, potentially reducing unnecessary invasive procedures.

Good fit: Ideal candidates are adults aged 18 and older who are referred for invasive coronary angiography due to suspected coronary artery disease.

Not a fit: Patients with recent acute coronary syndrome, severe claustrophobia, or contraindications to CMR or iodinated contrast may not benefit from this study.

Eligibility criteria

Inclusion Criteria:

* Patients aged ≥18 years
* Referred for invasive coronary angiography for investigation of chest pain

Exclusion Criteria:

* Recent acute coronary syndrome (\< 6 months)
* Previous coronary artery bypass grafting
* Severe claustrophobia
* Absolute contraindications to CMR - those with MR conditional or safe devices will be included
* Second-/third-degree atrioventricular block
* Severe chronic obstructive pulmonary disease
* Moderate-severe asthma
* Estimated glomerular filtration rate \<30 ml/min/1.73m2
* Women who are pregnant, breast-feeding or of child-bearing potential( premenopausal women)
* Contraindication to iodinated contrast
* Participation in a research study involving an investigational product in the past 12 weeks
