What this trial studies
This interventional surgical comparison enrolls men with long segment anterior urethral strictures and compares dorsal onlay buccal mucosal graft (BMG) urethroplasty to penile skin graft (PSG) urethroplasty. Eligible patients typically have strictures longer than 1 cm with spongiofibrosis and prior failed dilation or DVIU, while those with lichen sclerosus, prior hypospadias repair, or previous failed urethroplasty are excluded. Surgeons perform substitution urethroplasty using the assigned graft type and patients are followed for functional outcomes, complications, and stricture recurrence. The goal is to determine which graft yields better urinary function and fewer complications in this setting.
Who should consider this trial
Good fit: Men with long segment anterior (bulbar) urethral strictures over 1 cm with spongiofibrosis who have failed prior urethral dilatation or DVIU and meet surgical fitness criteria are ideal candidates.
Not a fit: Patients with lichen sclerosus, a history of hypospadias repair, or recurrent stricture after previous urethroplasty are excluded and unlikely to benefit from this comparison.

Inclusion Criteria: * Patients with long segment anterior urethral stricture that was defined as a stricture with a length of more than 1 cm with spongiofibrosis. * Patients with previous failed urethral dilatation or direct vision internal urethrotomy (DVIU) Exclusion Criteria: * Patients with lichen sclerosus * Patients with history of hypospadias repair * Patients with recurrent urethral stricture after failed previous urethroplasty
