What this trial studies

This type 1 hybrid-implementation effort partners community organizations with academic investigators to adapt and test outreach approaches using community-based participatory methods. In the Motivate phase, 400 Latino adults aged 50+ in San Francisco/Daly City are randomized across three arms (community health worker counseling plus text nudges, CHW-delivered RSV vaccine, or text nudges alone) and the primary outcome is RSV vaccine uptake. In a parallel Activate phase, 350 Latino adults aged 18–49 are randomized to CHW activation counseling plus text nudges versus text nudges alone, with the primary outcome the proportion of older contacts with whom participants discussed RSV vaccination. Both phases use culturally tailored interventions delivered in English or Spanish and measure implementation outcomes alongside effectiveness.

Who should consider this trial

Good fit: Ideal candidates are Latino/a/x or Latin American indigenous adults who are either (a) age 50+ who have not yet received the RSV vaccine, are fluent in English or Spanish, have a cell phone, and live or work in San Francisco or Daly City, or (b) age 18–49 who are fluent in English or Spanish and have at least one family member or friend aged 50+ in the U.S. they see or speak with regularly.

Not a fit: People unlikely to benefit include those already vaccinated against RSV, nursing home residents, those planning to move out of the area within a year, household members already enrolled in the aims, or anyone unable to provide informed consent.

Why it matters

Potential benefit: If successful, the approaches could raise RSV vaccination rates among Latino older adults and use family and social networks to spread vaccine information.

How similar studies have performed: Previous work shows community health workers and text-message reminders can increase adult vaccine uptake, but combining CHW outreach with social-network activation for RSV is relatively novel.

Motivate Aim Inclusion Criteria:

* Age 50 years or older
* Self-identify as Latino/a/x and/or indigenous groups from Latin America
* Eligible for RSV vaccination per current CDC/ACIP recommendations
* Fluent in Spanish or English
* Has not received the RSV vaccine
* Has a cell phone
* Lives or works in San Francisco or Daly City
* Able to provide informed consent

Motivate Aim Exclusion Criteria:

* Intent to move outside of San Francisco or Daly City in the next year
* Nursing home resident
* Household member participating in Aim 1 or 2
* Unable to provide consent

Motivate Aim Inclusion Criteria:

* Age 18-49 years
* Self-identify as Latino/a/x and/or indigenous groups from Latin America
* Fluent in English or Spanish
* Has \>1 family member or friend 50 years or older who they have seen or spoken to for \>15 minutes in the last 6 months and who lives in the United States

Activate Trial Exclusion Criteria:

* Household member enrolled in Aim 1 or 2
* Unable to provide consent
