What this trial studies

This study aims to address hypertension, which affects a significant portion of the adult population in the U.S., particularly in urban minority communities. It will adapt a successful intervention model previously implemented in Northern California to fit the unique needs of under-resourced neighborhoods in Chicago. The approach involves community-centered strategies, utilizing local clinics and churches to facilitate better blood pressure management among residents. By focusing on evidence-based practices, the study seeks to improve health outcomes and reduce disparities in hypertension rates.
